RELEASE
I, the undersigned, hereby release Eskrima LLC d/b/a PIMA ACADEMY, any instructors, St. Aloysius Elementary Academy and fellow students, be they individual or otherwise, from any liability whatsoever to my person and through me concerning activities related to Martial Arts and Fitness training, studying, practicing, or in the application of martial arts or fitness including, but not limited to, class activities, on and off premises, and sparring. I also state that I am in good physical condition and know of no reason why I cannot study and participate in martial arts. I understand that PIMA does not offer refunds. Additionally, I hereby authorize PIMA Academy Staff and St. Aloysius Elementary Academy staff and personnel to give permission to any physician or medical facility to treat my person if I am reasonably deemed at the time incapable or incompetent of giving such authorization and agree to bear the expense of any such treatment. I certify I have the legal authority to sign this release and so obligate those mentioned.
I also understand that in order to remain a member of Eskrima LLC d/b/a PIMA ACADEMY that all parents, guardians, and students must adhere to the code of conduct of Eskrima LLC d/b/a PIMA ACADEMY. If any parents, guardians, and/or students is in violation of the code of conduct, membership shall be terminated without refund.
Code of Conduct
Students:
- Respect instructors, other students, and parents/guardians
- Keep training area clean
- Avoid engaging in unsafe practices
- NO Hurtful speech
- DO NOT leave the mat without permission
- DO NOT call to parents/guardians from the mat
- Follow the class creed
- NO playing on the mat without permission
- NO food, drinks, toys, and shoes on the mat without permission
Parents/Guardians
- DO NOT step onto the mat without permission
- NO hurtful speech
- DO NOT call to children while they are on the mat
- DO NOT engage in activities on the mat without permission
- Please respect any instructions give by instructors
Please understand that training martial arts involves physical contact. Eskrima LLC d/b/a PIMA ACADEMY strives to maintain a safe training environment. However, due to the nature of our sport, accidents do occur. Sparring will causes bruises, cuts, and other minor injuries. Such things are unavoidable.
Uniform Policy
At PIMA Academy, our mission is to cultivate values such as honor, respect, discipline, focus, courage, and confidence in each of our students. One way we reinforce these principles is through the implementation of a standardized uniform policy, which fosters a sense of unity and reflects the discipline and respect we strive to instill.
All students will be required to adhere to the following uniform guidelines by wearing one of the below options for shirts and pants:

PIMA Academy shirt (short sleeve, long sleeve, or tank top)

Pants (PIMA Academy shorts, black gi pants, or red and black Doce Pares pants)

Red and black Doce Pares uniform

Belt (appropriate to their current rank)
Students who attend class without the required uniform will be subject to disciplinary action.
